Payment and Insurance

General Information

I have a fee for service practice.  Full payment is due at the time of service. Although my office does not submit bills to insurance companies, we provide you with a monthly billing statement that you may submit to your insurance company for reimbursement.  I do not participate in any insurance plans.  However, if you have a PPO plan that allows you to chose an out of network provider, after your deductible has been met, they may reimburse you directly.  You may use your HSA or flex spending accounts to pay for services. 

As insurance coverage and reimbursement are variable, we recommend that you contact your insurance company prior to scheduling your first appointment to verify the extent of your insurance coverage and determine whether or not they will reimburse you for my services.  

In addition, I am not a Medicaid or Medicare provider.  If you have Medicare Part B, prior to starting treatment, Medicare requires that you sign a "Private Contract" with me.  This contract informs you that you have the option to seek treatment with a provider who accepts Medicare.  The contract also specifies that neither you nor your doctor may submit claims or reimbursement requests to Medicare.  If you have a secondary insurance, they may or may not reimburse you for services provided under a "Private Contract".  We recommend that you check with your secondary insurance to determine if they will reimburse you for services for a provider who has "opted out" of Medicare.  I am not contracted with Medicaid and full payment of the session will be due at the time of service and you will not be able to request reimbursement for any fees or services paid.

Payment 

We accept cash, check, Visa, Master Card, Discover, and American Express at the time of service.  My billing system allows your credit card information to be stored securely.  If you do not pay by cash or check at the time of service, then your credit card is billed automatically at the time of service according to our fee schedule. 

Cancellation Policy

If you do not attend your scheduled appointment and have not notified us no later than 72 hours (3 business days) in advance, you will be charged the full amount for the missed session unless we are able to fill your scheduled time.
