I have a fee-for-service practice. I do not participate in any insurance plans and full payment is due at the time of service. However, if you have a PPO insurance plan that allows you to choose an out-of-network provider or see doctors with an out-of-network benefit, your insurance may reimburse you directly after your deductible has been met. As insurance coverage and reimbursement varies, we recommend that you first contact your insurance company prior to scheduling your first appointment to verify the extent of your insurance coverage and determine whether or not they will reimburse you for my services. You may use your HSA or flex spending account to pay for services.
Although my office does not submit bills to insurance companies, we will provide you with a monthly billing statement and superbill that you may submit to your insurance company for some reimbursement, depending on their guidelines. Please contact your insurance company for more information.
In addition, I am not a Medicaid or Medicare provider. If you have Medicare Part B, prior to starting treatment, Medicare may require that you sign a "Private Contract" with me. This contract informs you that you have the option to seek treatment with a provider who accepts Medicare, and specifies that neither you nor your doctor may submit claims or reimbursement requests to Medicare. If you have a secondary insurance along with Medicare, we ask that you check with that secondary insurance to determine if reimbursement is an option for providers who have “opted out” of Medicare. Full payment of the appointment session will still be due at the time of service.
If you have Medicaid, I am not contracted with them as a provider. Therefore, full payment of the session will be due at the time of service and you will not be able to request reimbursement for any fees or services paid out of pocket.

We accept cash, check, Visa, Master Card, Discover, and American Express at the time of service. My billing system allows your credit card information to be stored securely. If you do not pay by cash or check at the time of service, then your credit card is billed automatically at the time of service according to our fee schedule.
If you do not attend your scheduled appointment and have not notified us no later than 72 hours (3 business days) in advance, you will be charged the full amount for the missed session unless we are able to fill your scheduled time.
